Seems to be only two.  I have Form 605-C and instructions last revised May
23, 2002.

There are, however two certifications that worry me:

We must certify that we are not representatives of a foreign government.  As
John is from the planet Melmac this could be a problem

We must certify that the construction of the station for which this
application is being submitted will NOT be an action which is likely to have
a significant environmental effect (See 47 CFR SEctions 1.301 1.319 and
Section 97.13(a) - Do you think running in excess of 15KW could effect the
environment? I am not qualified to answer that and so am certifiying anyhow.
The clear cutting of the forest to make a good FD shouldn't be a problem
however.

I am almost sure you need 4 signatures - ask the ARRL for sure.  It used to
be two, but there was so much abuse, the FCC raised it to four.
