VASCM: VASCM 2016 Membership form

Hi All,

As ever, only $50 for the year (due at the beginning of the year, but we are a bit late this year). If you are renewing membership and have already filled out the form, no need to fill it all out again unless your information has changed. We ask new members to fill it out so we can get your information. You will find the membership form on the home page of the website here <http://www.vascm.org/>.   I have also attached the form.  Please complete as appropriate and send in with your 2016 dues.
