[TexasARDF] Entry fee, waivers?

     This is mostly for Mike...

     What should we do about entry fees and waivers?  I see the HOC now has
their entry form online.  Should we ask ARDFers to pay the same fee and sign 
the same form?  Or should we make up our own form and collect fees ourselves?
Or should we make up a separate page (like the Relays have their own page 3
in the HOC entry form as a supplement to the basic entry form?  I prefer the
latter idea, myself.
